The PACT RN will work closely with residents within our programs and encourage independence, competency, safety awareness, respect, dignity, and decision-making! Part-time or Full-time opportunities! 

  • Employee will assist and monitor activities of daily living

  • Monitors overall health and well-being.
    • Offer Monthly Nursing Assessments when needed
    • Yearly Primary Care Visits and when needed
    • Yearly Dental Care Visits and when needed
    • Yearly Eye Care Visits and when needed
    • Yearly Specialty visits and when needed
  • Assists with all aspects of medication management including connecting with Client’s VNAs.
  • Provides emergency service and crisis evaluation as needed.
  • Plans and provides opportunities for community activities.
  • Will review and be familiar with PACT orientation manual and Policy & Procedure Manual.
  • Will ensure all staff are following MAP Policies and Procedures and will follow these procedures.
  • Completes health and medication-related documents as individually indicated and in accordance with MAP regulations.
  • Under the direction of the team psychiatrist and in collaboration with other registered nurses on the team, to manage pharmaceutical and medical supplies.
  • Will coordinate, schedule and administer nursing assessments of client’s physical health, making appropriate referrals to community physicians for further assessment and teat and lastly, coordinate psychiatric treatment with medical treatment.
  • Will work weekends, second shifts and holidays as needed by supervisor.
  • Ensures that all sharps are accounted for and secured.
  • Provides safe and timely transportation in own vehicle, as needed for medical appointments, recreational activities, shopping, errands, etc.

The Program for Assertive Community Treatment (PACT) program is a highly integrated outreach and 24/7 residential program that helps five individuals meet their goals and reduce hospitalizations. PACT provides clinical wraparound support in the community and in our group residential program to perpetuate recovery and wellness for the individuals served. The PACT team is forensic-focused and designed to reduce arrests and incarcerations.

Job Requirements

Experience and Training:

  • Must maintain current RN licensure and certifications.
  • ** Will attend competence in motivational interviewing.
  • Registered Nurse: Licensed in Commonwealth of Massachusetts.
  • Must have a valid driver’s license and a reliable car available for working hours.
  • MAP certification is not required.
  • Must be committed to consumer empowerment, growth, human rights, and community integration.
  • Must be well organized with good interpersonal and verbal skills.
  • Must be familiar with internet, word processing and email systems.
  • 20 hours per week (flexible schedule to be determined upon hire, may need to do occasional weekends)

  • Location: Holyoke but will train in Springfield for a few months

  • In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.
